I think he's actually very confident in himself moreso because of his personality rather than appearances and it goes for how he views others too. He cares more about their values and their loyalty to the league above all else. Anyone who harbours any affection towards him should do so for the same reasons as well, if not then I don't think he would be interested anyway so I don't think there's any reason he would be worried about how he looks

so, you know the scene where Shoto asked Deku "are you All Migh's secret live child or something?". The thing is, why would Shoto think that? And here's my theory: Shoto used to watch soap operas as a child. Why would I think that? Because the "secret love child" trope is very common in soap operas (example: in Gilmore Girls, which some consider a soap opera, a character had a secret love child with his ex gf, which causes him to break up with his fiancee). But now you may ask: why would Shoto watch soap operas? And here's the thing: I don't thing he did that by his own initiative, I think Fuyumi made him watch them with her. But why would Fuyumi watch soap operas? Glad you asked.

As you can see in her character profile (made by Horikoshi obv), her favorite things are soap operas.

So, my theory is that Fuyumi and Shoto watched soap operas together as kids
